Decentralization of Clinical Trainings

 

During the IFPS phase 1 and initial period of IFPS II, SIFPSA had been involved in providing various types of clinical trainings to doctors and para-medicals in Public Sector. During these trainings Medical Colleges were involved as training centers. The medical colleges were provided assistance in terms of strengthening of infrastructure, training materials, printing of training manuals, contractual staff etc.

In order to create a pool of large number of trained service providers in a short period there was a shift in training strategy, the trainings to be conducted at divisional level women hospitals as Divisional Clinical Training Centres (DCTCs) instead of medical colleges. However, the medical colleges are involved in imparting Master Trainers Training to doctors of divisional clinical training centers, who in turn will train doctors and para medicals posted in the districts within the division.

The DCTCs have been established at Allahabed, Azamgarh, Kanpur Nagar, Moradabed, Meerut, Saharanpur, Mirzapur, Agra, Jhansi & Varanasi., remaining 8 divisional women hospitals will be strengthened in near future. The Strengthening of DCTCs done in the existing infrastructure of district women hospitals after gap assessment in terms of infrastructure (Minor renovation repair), equipments, training materials, audio/video aids etc. and provision of the same as per need.

Master trainers training provided to doctors of District hospitals (DCTCs) for Abdominal Tubectomy at Jhansi, Varanasi and Agra and for lap ligation for doctors of DCTC Allahabed, Azamgarh, Kanpur Nagar, Moradabed, Meerut, Saharanpur, Mirzapur,at Merdical Colleges.

For IUCD trainers training for 10 DCTCs have been conducted at Veeranga Avanti Bai Hospital, Lucknow, through the Master Trainers trained at GOI level. These trainers has trained the team as district trainers in forty districts, The IUCD training is likely to be initiated in near future.

 

Clinical Training Standards

 
Since the clinical trainings have been decentralized to divisional level women hospitals from medical colleges, SIFPSA has introduced quality standards for Divisional Clinical Training Centres with Technical Assistance from Jhpiego (An affiliate of Johns Hopkins University) through USAID. A self assessment system (Tools) on various training aspects on quarterly basis by Incharge DCTC and other officials is being introduced in near future

Computer Based Training for improving training skills of trainers developed by Jhpiego at DCTC is also being initiated shortly.

 

Skilled Birth Attendant training for Medical Officer

 
Government of India has developed 10 days training package for SBA training of Medical Officers. SIFPSA has piloted this training package through SIFPSA funds and later the training is being undertaken through NRHM funds. Till date 165 doctors have been trained in UP. This training is being imparted to Medical officers posted at 24x7 sites and preference is given to districts where the SBA training of Paramedical has been completed or ongoing.
 

Clinical Trainings through NRHM Funds

 
SIFPSA has been designated as nodal agency for certain type of clinical training. The funds for conducting these trainings have been booked under the NRHM PIP. The Laparoscopic Tubal Ligation, Minilap / Abdominal Tubectomy and IUCD training is being undertaken at DCTCs. The SBA training for doctors is conducted at Medical Colleges, and NSV at Centre of Excellence at Department of Urology, CSMMU, Lucknow and recently extended at three medical Colleges viz. Allahabad, Meerut and Kanpur as Satellite Centres for NSV.

Re-designing of Subcentre facility in Uttar Pradesh

 

With the aim to fulfill the felt need to improve the design structure of SC buildings after identification of gaps in the existing design structure of SC buildings & sharing of experience & problems with service providers & supervisors, project is piloted on 2 blocks ( single storey SC at Natkur, double storey SC at Amethia Salempur) of district Lucknow to create a more client friendly service site in order to provide quality RCH services to have a positive impact on the health indicators of far flung & remote villages with the following objectives :

  • To provide adequate space for specific procedures & objectives.
  • To increase visibility of SC facility for increase uptake of services at sub centers & 24 hour availability of service provider at SC site.
  • To create 2 different SC design models as per need of the terrain.
  • Chief Medical Officer of Lucknow will be the implementing agency.

Till date, single storey SC building at Natkur & double storey SC building at Amethia Salempur is completed. ANMs will be posted at these sub centers by CMO to make the 2 subcenters functional & user friendly, with an expected outcome of an acceptable suitable design with increased uptake of quality RCH services & increased client satisfaction.
